It was United’s earliest exit from the tournament in more than ten years, underlining their miserable 2024/25 campaign.
And while the Red Devils were overwhelming favourites on paper, they carried the added burden of going into the match without a win from their first two Premier League games.
However, Grimsby silenced Ruben Amorim's side by striking first in the 22nd minute, with Charles Vernam squeezing a shot past Onana at his near pos, per Yahoo Sports.
Just eight minutes later, the under-pressure goalkeeper was culpable again, failing to deal with a corner that dropped for Tyrell Warren to tap home into an empty net as seen in the video below.
United clawed their way back with late goals from Bryan Mbeumo and Harry Maguire to force a penalty shootout, but Onana saved only one of 12 spot-kicks as Grimsby held their nerve to clinch a famous victory.

@AlanShields12, questioned the goalkeeper's commitment to the cause, writing:
"Onana should never play for Utd ever again, sure he is doing it on purpose at this point."
@centredevils fumed:
"SELL ANDRE ONANA IMMEDIATELY, DONT EVEN LET HIM ON THE BUS."Summing up Grimsby's hopes of advancing, @bethTmufc wrote: "All they have to do is shoot … Onana".
Onana has reportedly drawn interest from West Ham ahead of the summer transfer deadline, sparking speculation over a possible exit from Old Trafford.
Yet his dreadful showing against Grimsby may cool that pursuit, while intensifying United’s push for a new goalkeeper.

The Cameroon shot-stopper wasn’t the only one in the firing line, as furious fans also turned on Ruben Amorim after the Carabao Cup humiliation, piling even more pressure on the under-fire Portuguese coach, with growing calls for his dismissal.
Amorim is under serious scrutiny, with just one win in his last seven games and a win rate barely above 37% since taking charge, per the BBC.
